Please complete this form to request an interdistrict school transfer due to the location of your childcare provider.

Please read these instructions before completing the Election to Attend form. 

This form may be used in a situation of divorce or separation. If at least one parent resides within the Duneland School Corporation (DSC) attendance boundaries, the parents may elect to have their student(s) enroll in DSC.

Please note: This form, along with the required residency verification documents, must be submitted before the first day of July each school year.
